How Commander Health works, from upload to signature

Commander Health works from your records only, with no calls or video visits. You upload your records for a free screen. If the evidence is there, you pay a flat fee ($500 for a nexus letter, $200 for a DBQ, $550 for both), a licensed clinician reviews your file, and you get an email when your signed document is ready. If your records do not support it, the clinician declines and you get your money back.
Step by step
Every service follows the same path. The fee is flat: $500 for a nexus letter, $200 for a DBQ, or $550 for both.
- Upload your records for a free screen. No account is needed. The screen checks whether your records hold what the document needs.
- If they do not yet, the screen tells you what is missing and you pay nothing.
- If they do, you approve a summary of your records for the clinician and pay the flat fee for the service you chose.
- A licensed clinician reviews your records. If something is missing, the clinician asks you by email.
- If your records support it, the clinician signs and you get an email when your signed document is ready to download. If they do not, the clinician declines, your case is closed with a full refund to your original payment method, and your case status page shows the reason.
If the clinician declines
The clinician can say no. A licensed clinician signs only when your records support the opinion, and paying the fee ($500, $200 or $550) does not change that.
If the clinician cannot support it, your case is closed and a full refund goes to your original payment method. The clinician records the reason, and your case status page shows it. If you have questions, reply to the email you received.
Records only, by email
There are no appointments, calls or video visits. The clinician reads your records, and any follow-up question comes to you by email. Records are handled under signed HIPAA Business Associate Agreements.
Prices
Published prices for a private nexus letter ran from about $500 to over $3,000 in September 2026, and many add a separate $195–$395 records-review fee before the letter. Commander Health's flat $500 includes the clinical review of your records.
- Nexus Letter: $500 flat. Full clinical review of your medical records; Nexus letter signed by a licensed clinician when their review supports it; One free rebuttal letter if the VA denies your claim.
- DBQ Completion: $200 flat. Full clinical review of your medical records; VA DBQ completed and signed when the clinician's review supports it.
- Nexus + DBQ Bundle: $550 flat. Everything in both services in one engagement; One records review covers the nexus letter and the DBQ; One free rebuttal letter if the VA denies your claim.
What Commander Health does not do
- Physical conditions only: Commander Health does not write PTSD or other mental health opinions or DBQs.
- Not a filing service. Commander Health does not file claims and is not a VSO, a law firm, or a VA-accredited representative. Free help filing a claim is available from a VA-accredited Veterans Service Organization.
- Not a guarantee. Clinical review never guarantees an outcome. A clinician signs only if your records support the opinion, and VA decides the claim.
