One review of your records by a licensed clinician covers both: the nexus letter on whether your physical condition is connected to service, and the DBQ on how severe it is. The clinician signs only when your records support it. Money back if we cannot complete your case.
What the $550 includes
One flat fee of $550, compared with $500 for the nexus letter and $200 for the DBQ on their own. That is $150 less, because one records review does the work of two.

When a claim needs both
A nexus letter answers whether a condition is connected to service. A DBQ records how severe it is so VA can rate it. A new claim for a physical condition VA has not connected yet can need both: the opinion to connect it and the findings to rate it.
If VA already connected the condition and the question is only the rating, the DBQ alone may be enough. If the screen finds the records hold an opinion but not the DBQ measurements, it says so before you pay.
How it works
From upload to signed documents, for a flat $550:
- Upload your records for a free screen. No account is needed. The screen checks whether your records hold what the document needs.
- If they do not yet, the screen tells you what is missing and you pay nothing.
- If they do, you approve a summary of your records for the clinician and pay the flat $550.
- A licensed clinician reviews your records. If something is missing, the clinician asks you by email.
- If your records support it, the clinician signs and you get an email when your signed document is ready to download. If they do not, the clinician declines, your case is closed with a full refund to your original payment method, and your case status page shows the reason.
What it is not
- Physical conditions only: Commander Health does not write PTSD or other mental health opinions or DBQs, and completes only VA's public DBQs.
- Not an exam or an appointment. There are no calls or video visits; the clinician works from your records, and any follow-up comes by email.
- Not a filing service. Commander Health does not file claims and is not a VSO, a law firm, or a VA-accredited representative. Free help filing a claim is available from a VA-accredited Veterans Service Organization.
- Not a guarantee. Clinical review never guarantees an outcome. A clinician signs only if your records support the opinion, and VA decides the claim.
